Vermont Gov. Jim Douglas said President Barak Obama has selected Vermont as one of eight states to participate in a demonstration project that is modeled after, and will strengthen, the groundbreaking Vermont Blueprint for Health.
As part of the demonstration project, the federal government will provide Medicare funding to better coordinate care, lower costs and improve health outcomes for patients, Douglas said. This is a first for the federal Medicare program.
Currently, Vermont's Blueprint for Health is funded by state general fund tax revenue, the state's Medicaid program and Vermont's major commercial insurers.Under the demonstration project, Medicare funding will be used to advance Vermont's existing Blueprint program, a project that supports integrated and proactive primary care for patients that save money and improve quality.
